Specialists for the first steps back home

Physical and/or mental illness can cause anxiety, mistrust, disorientation, lack of motivation and similar feelings. These feelings can make it difficult for patients and their social environment to imagine returning home from the hospital. Transitional care offers help and support to older people so that they can live independently at home again after being discharged from the Tauernklinikum hospital.

Supported. Accompanied. Strengthened.

Experience shows that the familiar surroundings of home and the usual daily routine promote meaningful activity. This leads to a significant improvement in mental, physical and intellectual performance. Transitional care therefore accompanies and supports elderly patients after an inpatient stay to enable them to be discharged to their own homes. Particular emphasis is placed on promoting and maintaining the patient's independence.

For a successful transition into everyday life

Transitional care, a free service offered by Tauernklinikum in cooperation with the province of Salzburg, acts as a bridge between the hospital and home. This form of care provides a sense of security for patients and their relatives, as a personal contact person is available.

Contact with the transitional care department begins at the hospital itself, with intensive discussions between patients, carers, treating physicians, therapists and relatives. The goal is to work together to find a suitable way for the patient to return home.

Therapeutic discharges home

Therapeutic visits home are carried out with the patients while they are still in hospital. The aim of these visits is to assess the patient's current physical abilities in their home environment, the current care options available at home and to determine what measures should be implemented to enable a return home.

For patients, these outings often represent an emotional, mental and physical challenge. However, they help to better assess the real-life situation and, as a result, make targeted decisions.

Accompanied discharge home

On the day of discharge, our transitional care nurses personally accompany patients home. Once there, we provide support with organisational issues and help them settle back into their familiar surroundings.

Aftercare at home

Even after discharge, we are there for our patients – with comprehensive aftercare services provided directly in their home environment:

  • Initial assistance to help them find their feet again at home
  • Empowering and anxiety-relieving conversations
  • Biography work to activate personal resources
  • Everyday training to promote independence
  • Medical treatment as required
  • Support in organising further social services, if required
